Disability Alert Wallet Cards
These free, printable wallet cards help people with intellectual and/or developmental disabilities (I/DD) quickly share critical information with first responders during an emergency. Cards cover eight disability types including autism, epilepsy, mobility disabilities, vision and hearing disabilities and I/DD. Each card includes space for a name and emergency contact, plus guidance for first responders on how to help.
For providers: If you support individuals with I/DD, take our interactive course Disaster Preparedness Guidebooks for Providers Supporting Individuals with I/DD in the Disaster Services section to build personalized preparedness plans together. You can:

- Print the PDF version of the guidebook for use with individuals and families
